I have often wondered about this issue. Unfortunately, my knowledge of
the subject matter is rather scattered so I don't have enough information
to make any kind of intelligent, well thought-out response at this time.
However, I will point out that this is not just a problem for
accessibility extensions to operating systems, X-windows has long been
known to pose potential security compromises. It is possible, as I
understand it, tfor a user to capture another users keystrokes and
screens when using X. Again, I don't understand all of the
ramifications, but this problem has been known for quite some time,
especially in military installations.

There is much to learn.
