Duplicates happen because multiple people send them to the list.
in the case sited, I sent the message to the list because I didn't realize
that Curtis Chong figured out what happened and was able to resubmit the
message. I'm not sure what can be done to prevent this sort of thing as we
have no centralized authority. Does the list as a whole feel we need to
address this issue?
